2-1-1 San Diego is hosting a five-part webinar series that will provide a deep dive into each section of their recently released CIE Toolkit: Collaboration and Cross-Sector Data Sharing to Create Healthier Communities.
The final webinar in this series will provide an overview of sustainability and how communities can get involved in the larger movement. The session will initially focus on gaining a clear understanding of one-time and ongoing costs, outlining potential business models, and exploring strategies to create value propositions for key stakeholders and potential funding streams. The presentation will also explore our collective role in building the ﬁeld of health care-social service collaborations by conducting research that impact of social determinants on population health, and championing eﬀorts to standardize care with an eye toward achieving equity and social justice.
We will also provide a sneak peek into the upcoming CIE Summit, to be held in San Diego on April 24 – 26, 2019!
